Introduction: Smoking is the major risk factor for development of chronic lung disease and airway malignancy. The biomarkers onset early progression hindered by accessibility primary tissue in lungs, so there a need to evaluate alternative sites surrogate biomarkers. harmful effects seen lower distal airways are also mirrored nasal epithelium as one disease.
 Objective: To study correlation between mucosal cytology, mucociliary function, airflow function among current smokers, never former smokers.
 Methods: Cross sectional, observational from tertiary care hospital.105 subjects were randomly distributed on basis smoking pattern into 3 groups, smoker, smoker smoker. Nasal cytology assessed saccharin transit time test (STT), peak inspiratory flow(PNIF) ciliated cells & goblet cell ratio. was spirometry.
 Results: increase statistically significant (p <.001) smokers compared smokers. mean count group when (p.023) while change cell/ ratio remained insignificant. decrease FEV1/FVC 0.036) both Conclusion: reduced this reduction permanent cessation does not improve function.
